Who We Serve

Ages 18 and Older

Ability First Care supports individuals with intellectual and developmental disabilities (IDD), ages 18 and older, who need daily assistance in a calm, home-like setting. We provide residential support through the Texas Home and Community-Based Services (HCS) Waiver Program and private pay.

Our group homes are intentionally small, creating a comfortable environment where individuals can feel safe, seen, and supported. Our person-centered approach respects personal choice, fosters independence, encourages meaningful relationships, and supports each individual in participating more fully in everyday life and their community.

Our Services

Centered Around the Individual

Every service is tailored to the individual we support and guided by their Person-Directed Plan (PDP) and Individual Plan of Care (IPC). Supports are designed around each individual’s goals, preferences, abilities, and identified needs, with services adapted as needs and circumstances change.

Residential Support

24-hour residential support in a safe, structured, home-like environment. Staff provide supervision, household support, community participation, and assistance with routines based on each individual’s authorized services and person-centered plan.

ii.

Personal Care & Daily Living

Assistance with personal care and activities of daily living, including grooming, bathing, dressing, meals, and other daily routines while promoting dignity, choice, and independence.

iii.

Life Skills Development

Support with developing and maintaining practical skills for everyday living, including household routines, decision-making, self-advocacy, and skills that promote greater independence.

iv.

Community Integration

Support to participate in meaningful community activities, pursue personal interests, develop relationships, and build confidence and independence in everyday life.

v.

Behavioral Supports

Individualized behavioral support coordinated with qualified professionals and aligned with the individual’s needs, preferences, and person-centered plan.

vi.

Transportation & Community Access

Transportation support for authorized appointments, community activities, employment-related activities, and other approved destinations that support participation and independence.

vii.

Medication Support

Assistance with medication routines and medication-related supports, as authorized and permitted under applicable requirements.

viii.

Individualized Skills & Socialization Support

Support for individuals attending authorized Individualized Skills & Socialization (ISS) services through qualified providers.

ix.

Coordinated Professional Supports

When authorized and needed, Ability First Care coordinates with qualified professionals and providers to support the individual’s needs, including nursing, behavioral, and therapeutic services.

We welcome referrals and inquiries from families, guardians, service coordinators, case managers, LIDDAs, and healthcare professionals seeking residential support for individuals with intellectual and developmental disabilities.

We aim to respond within 48 hours to discuss and determine the appropriate next steps.

Secure Documentation

If additional information is needed, we will provide instructions for securely submitting required documentation.

Review & Next Steps

Our team reviews the information and communicates the next steps in the process.
